The Immunizations webpage is a method of sharing information to parents, students, staff, and communities about immunization requirements for school attendance in accordance with the Official Code of Georgia O.C.G.A § 20-2-771. Stay current of immunization resources to ensure compliance for attending school and to help APS promote a healthy and safe environment for all APS students. Information is updated and aligned with the current recommendations of the Advisory of Committee on Immunization Practices (ACIP), the American Academy of Pediatrics (AAP), and the American Academy of Family Physicians (AAFP).
By state law, a valid certificate of immunization (Form 3231) or a notarized statement of religious exemption (Form 2208) Religious Exemption Form 2208 - ENGLISH or Religious Exemption Form 2208 - SPANISH is required for enrollment. The certificate of immunization must be completed by the health department, a licensed healthcare provider, or your private physician. A valid Form 3231 must be marked with either "Date of Expiration" or as "Complete for School Attendance." (A certificate marked with a "Date of Expiration" expires on the date indicated, and a current certificate must be submitted within 30 days of expiration.) Expired certificates will not be accepted for enrollment. A medical exemption, if applicable, should be noted on Form 3231 with a current date of expiration.
For students entering from out-of-state schools, please contact your local Health Department or a Georgia licensed physician to have immunizations transferred to the Georgia Immunization Certificate (Form 3231). You will need to have your immunization record from your state in order to transfer immunizations to Form 3231. All requirements apply to students transferring from out-of-state schools to Georgia schools.
All Kindergarten, sixth-grade students, seventh-grade students, students new to Georgia, students returning to a Georgia school after being away for more than one school year, and students enrolling after attending a private school must have a valid Certificate of Immunization.
